L’estrazione di un dente del giudizio è un’operazione molto comune: il loro sviluppo può dare il via a una serie di problematiche che ne rendono necessaria la (spesso) temuta estrazione.

Ma quando e perché vanno tolti i denti del giudizio?

Scopriamo di più insieme.

 

Cosa sono i denti del giudizio?

I terzi molari, conosciuti come denti del giudizio, sono gli ultimi denti definitivi che spuntano a completare le arcate dentarie. Rappresentando l’ultimo stadio della dentizione permanente, occupano lo spazio rimasto nella posizione più interna della bocca. Non tutti sanno che si tratta degli elementi dentari meno utili a livello funzionale, legati prevalentemente a uno stadio evolutivo passato. Ecco perché non è detto che spuntino sempre e tutti.

Come abbiamo accennato però la loro comparsa provoca spesso fastidi e può influire negativamente sul resto della dentatura. Ecco perché spesso è necessario procedere all’estrazione del dente del giudizio.

 

Perché si chiamano denti del giudizio?

I denti del giudizio sono una componente ereditata dai nostri antenati che sopravvivevano mangiando cibi crudi e quindi difficili da mordere. Alcuni hanno la fortuna di non presentare i denti del giudizio (agenesia) proprio perché col tempo le nostre abitudini alimentari sono cambiate, i denti del giudizio hanno perso la loro utilità e la nostra dentatura si è evoluta insieme a noi.

 

I problemi legati all’eruzione dei terzi molari

Purtroppo i denti del giudizio si situano in una posizione scomoda e poco spaziosa. Per cui è solito notare la presenza di denti del giudizio storti, non completamente erotti o bloccati all’interno della gengiva. Tutte condizioni potenzialmente pericolose per il resto della dentatura poiché potrebbero provocare disallineamenti dentali, infiammazioni alle gengive o danni ai denti adiacenti.

Gli odontoiatri sono così soliti distinguere:

  • Denti del giudizio ritenuti: non hanno la radice completamente formata per cui potrebbero ancora uscir fuori.
  • Denti del giudizio inclusi: il dente del giudizio è incluso nella mascella con una radice già formata. Talvolta non presenta sintomi, altre invece infiamma la sede gengivale portando gonfiore e mal di testa. Un dente del giudizio incluso può comportare diverse problematiche. La sua radice, potrebbe continuare a svilupparsi intaccando il nervo mandibolare con il rischio di frequenti dolorose nevralgie. Ecco perché in questi casi si ricorre generalmente all’estrazione dente del giudizio.
  • Denti del giudizio semi inclusi: una parte del dente è inclusa mentre l’altra fuoriesce della gengiva e potrebbe spingere i denti contigui. Inoltre la posizione nascosta del dente del giudizio semi incluso non permette una corretta igiene orale favorendo così i processi cariogeni e le infiammazioni gengivali acute con formazione di tasche parodontali.

 

Quando diventa necessaria l’estrazione denti del giudizio?

I problemi legati alla nascita dei denti del giudizio possono nascere nel tempo gradualmente o senza preavvisi.

Per questo diventa necessario rivolgersi al dentista di fiducia già dai primissimi sintomi o avvisaglie. Un intervento tempestivo potrebbe evitare complicazioni dolore o addirittura (in alcuni casi) il ricorso all’estrazione.

In generale si procede a un’estrazione denti del giudizio quando:

  • Si sviluppa una carie nel dente del giudizio: considerando l’inutilità ai fini masticatori non ha senso effettuare una devitalizzazione del dente, per cui si preferisce eliminarlo.
  • È presente un’infezione: se sono presenti ascessi o cisti molto dolorose che compaiono ciclicamente, meglio togliere il dente del giudizio.
  • Il dente del giudizio si scheggia: se il dente si rompe non ha alcun senso ricostruirlo né per fini estetici né funzionali.
  • Il terzo molare è mal posizionato: se il rischio che il dente del giudizio provochi il disallineamento o la lesione degli altri elementi dentari è alto, è generalmente consigliato rimuoverli per prevenire complicazioni future.
